An Alternative to JAK Inhibitors May Be on the Way for Alopecia Areata

Alopecia areata has a frustrating pattern: hair falls out, sometimes grows back, then falls out again, often with no clear answers for people who’ve already tried the standard treatments. Now there’s a new name to know when it comes to treatment. Bempikibart, an experimental antibody treatment, is showing encouraging early signs of regrowth, even in patients whose alopecia didn’t respond well to JAK inhibitors, one of today’s most common alopecia areata treatments. As reported by Dermatology Times, Q32 Bio recently announced 36-week results from Part B of its SIGNAL-AA phase 2a trial.

What the Trial Found

The open-label trial enrolled 33 patients with severe or very severe alopecia areata, defined as 50 to 100 percent scalp hair loss, with a current episode lasting no more than four years. Patients started on weekly injections of bempikibart for four weeks, then moved to injections every other week through week 36. Enrollment actually ran higher than planned; demand from patients pushed it past the original target.

At week 36, bempikibart reduced participants’ Severity of Alopecia Tool (SALT) score, a measure of scalp hair loss, by an average of 35.3 percent from baseline. Forty percent of patients reached a SALT-20 response, meaning 80 percent or more scalp hair coverage. Notably, more than a third of participants had already tried an oral JAK inhibitor without lasting success, and the drug still worked for many of them.

Why This Could Matter for JAK Inhibitor Patients

Boston dermatologist Arash Mostaghimi, MD, associate professor of dermatology at Harvard Medical School, said the findings reinforce bempikibart’s potential as an alternative to JAK inhibitors. “The robust efficacy data in a population that includes JAK inhibitor-experienced patients, combined with a differentiated safety profile, demonstrate the potential for bempikibart to be a first-line treatment for alopecia areata,” Dr. Mostaghimi said. “For patients and prescribers seeking an effective and safe alternative to JAK inhibitors, these findings are encouraging and merit further clinical advancement.”

Safety and What’s Next

The most common side effect was mild injection site reactions, which mostly resolved within a day. No serious adverse events turned up in the trial. Bempikibart already holds FDA Fast Track designation, granted in March 2025, and Q32 Bio says the data supports moving toward a registration-directed program in the first half of 2027.

What This Means for You

Bempikibart hasn’t been approved yet, and larger trials still lie ahead. But for people whose alopecia areata hasn’t responded well to existing treatments, results like these are reason for cautious optimism. It’s one more potential option, and one more reason to bring up hair loss treatment history with your dermatologist as new therapies move through the pipeline.
