Dermatologists Share 4 Solutions for Stubborn Chest Wrinkles

The chest, or décolletage, often reveals signs of aging sooner than we expect. As Washington, D.C. dermatologist Nina Hartman, MD explains, this area is especially important to address early. “Like the face and neck, the décolletage is chronically exposed to environmental aging—most notably ultraviolet (UV) radiation—and therefore tends to show signs of aging relatively quickly. Treating the face, neck and chest together is essential for achieving a natural, harmonious appearance.”

Prevent Damage With Sunscreen

Chest wrinkles often start with chronic sun exposure, making daily SPF the most important first step.

Dr. Hartman stresses that “daily broad-spectrum SPF applied to the chest—with reapplication when outdoors—is critical.” She also notes that relying solely on clothing isn’t enough: “UV radiation can penetrate certain fabrics, so relying on clothing alone is not a foolproof strategy. While sun-protective (UPF-rated) clothing offers more reliable coverage, sunscreen remains a cornerstone of prevention.”

San Diego dermatologist Azadeh Shirazi, MD agrees, calling chest SPF “non-negotiable.” She says, “Many people don’t realize the skin on the chest is thinner than the face, so UV rays penetrate more easily and accelerate collagen breakdown, which is why these lines form and linger.”

Once daily protection is in place, targeted skin care can help strengthen the chest’s thin, UV-sensitive skin.

Dr. Shirazi recommends extending your existing routine downward: “Topical retinoids and vitamin C [should be] part of your skin-care routine that should extend down to your neck and décolletage. Retinoids boost cell turnover and collagen production, while vitamin C protects against oxidative damage and supports brighter, healthier skin.”

Dr. Hartman agrees, emphasizing consistency and restraint. “In addition to sun protection, anti-aging topicals similar to those used on the face and neck should be incorporated into the chest routine,” she says. “Retinoids or retinol used in the evening help stimulate collagen production and improve the appearance of fine lines, wrinkles and crepiness, while also aiding in pigment correction.”

That said, she cautions against rushing results: “I recommend a ‘low and slow’ approach—starting with a low-strength retinol used infrequently and gradually increasing as tolerated, allowing the skin time to adapt.”

Supplement With Silicone Patches

If you’re a devoted side-sleeper—or frequently wake up with chest creases—silicone patches can offer extra overnight support. “I recommend silicone sheets at night to reduce repetitive creasing from side sleeping,” says Dr. Shirazi. “They’re not perfect, but they can meaningfully soften lines over time.”

These medical-grade silicone patches are reusable and help smooth the area while preventing further creasing. Apply them to clean, product-free skin and wear overnight (or for at least an hour if you’re short on time) for visible, temporary smoothing benefits.

Consider In-Office Solutions

For wrinkles that don’t respond to at-home care, in-office treatments can deliver deeper, more noticeable improvement. Both experts note that the chest often responds best to a combination of collagen-stimulating and resurfacing procedures.

Dr. Shirazi favors a layered approach: “For stubborn chest wrinkles, I like a multimodal approach: resurfacing lasers such as Halo or ProFractional to smooth the skin’s surface and trigger collagen remodeling, combined with biostimulators like Sculptra or hyperdiluted Radiesse, which act as a scaffold and signal fibroblasts to produce new collagen over time. RF microneedling adds another layer by tightening and strengthening the dermis, which is especially helpful for crepey texture and movement-related lines.”

Dr. Hartman highlights the value of light- and laser-based treatments for both wrinkles and sun damage:  “Laser and light-based therapies are highly effective for treating photodamage, including brown spots, uneven tone and fine lines,” she says. “IPL and other broadband light devices target pigmentation, while fractional lasers such as Fraxel improve both discoloration and texture by stimulating collagen remodeling.”

For deeper lines, she also turns to biostimulatory injectables: “Calcium hydroxylapatite (Radiesse) and poly-L-lactic acid (Sculptra) stimulate long-term collagen and elastin production, leading to improved skin quality and softening of deeper wrinkles.”

Ultrasound tightening is another option. “Microfocused ultrasound devices, such as Ultherapy, are FDA-cleared for the treatment of chest wrinkles and can also help improve mild skin laxity in this area,” Dr. Hartman notes.
