Where to Start If You’re Considering GLP-1 Weight-Loss Treatment

With GLP-1 medications now part of the weight-loss conversation, many patients aren’t sure where to start. Should you see an obesity specialist, or is telehealth a good option? And with so many new providers entering the space, it can be hard to know who to trust. Experts say finding the right care team can make all the difference in both results and safety.

Start With Your Primary Care Doctor

“The reality is that for most people, I would start by talking to your primary care doctor,” says San Diego internist and obesity specialist Eduardo Grunvald, MD. “See if they feel comfortable considering treatment. If not, that’s when you can go beyond that and look for an obesity-medicine specialist or a reputable telemedicine option.”

He adds that this initial conversation can also uncover whether other health conditions need attention before beginning treatment. “Your doctor already knows your history, medications and lab trends,” he says. “That context is valuable when deciding if a GLP-1 is appropriate.”

Look for an Experienced Provider

New York internist Amanda Kahn, MD says one of the most important things patients can do is choose a provider who bases decisions on data, not trends. “Ask what protocols they follow for lab testing and monitoring,” she advises. “GLP-1 therapy should be tailored to your health profile and paired with a plan that focuses on long-term metabolic improvement, not just rapid weight loss.”

She notes that transparency is key: “A good provider explains why a medication is being chosen, what to expect during titration and how success will be measured beyond the number on the scale.”

When Telehealth Makes Sense

Telemedicine has opened access to GLP-1 treatments for people who might not have a specialist nearby. “If you’re generally healthy, a telemedicine option can work well,” says Dr. Grunvald. “But if you have more complex health issues, like if you’re on blood-pressure medication, insulin, or have heart disease, then it’s better to start in person. Those situations often require physical exams and medication adjustments.”

Chicago internist and bariatric specialist Neha Shah, MD agrees that convenience can be helpful, but only when quality controls are in place. “You should always know who your prescriber is, have access to follow-ups and receive guidance on nutrition and exercise,” she says. “GLP-1s work best when they’re part of a broader health plan, not a quick-click prescription.”

Check Credentials and Oversight

Dr. Shah cautions against platforms that minimize medical review. “If a program doesn’t require labs, it’s a red flag,” she explains. “Any legitimate prescriber will want to see your metabolic and hormonal markers before starting medication.”

She adds that board certification in internal medicine, endocrinology or obesity medicine is a strong indicator of expertise. “Credentials matter,” she says. “So does access, make sure you can reach your provider between visits if something doesn’t feel right.”

Prioritize Ongoing Support

Dr. Kahn emphasizes that consistent care is what helps patients sustain results. “Medication is only one piece,” she says. “Regular follow-ups, guidance on nutrition, sleep and movement and managing side effects early are what make the difference between short-term success and long-term change.”

When to See a Specialist in Person

“If you’re someone with multiple medications, cardiovascular disease or insulin-dependent diabetes, I’d prefer to see you in person,” says Dr. Grunvald. “That first visit allows for a more thorough evaluation and we can make sure other conditions are managed safely as you lose weight.”
